Dental Membership

Dental membership plans come with other advantages that most dental insurance plans do not offer.  For example dental membership plans do not have exclusion clauses, missing tooth clauses, claim forms, deductibles, waiting periods, or annual maximums.  If you read the fine print of dental insurance policies they typically exclude certain services from coverage and frequently have waiting periods prior to taking advantage of coverage.  Dental insurance also has annual coverage limits, which can be frustrating for anyone who needs dental work, which exceeds the annual limit.  Unfortunately it is not always possible or advisable to delay dental work until the next year.  Dental membership plans do not come with any limits on maximums.   Savings are applicable for all procedures and products provided during any given period of time. 

Membership plans are a value and simply transparent.

• The Dental Membership Plan is not dental insurance. The program is offered to individuals and families who do not have insurance, or who are able to inactivate their insurance with their employer.
• Dental Membership cannot be used in conjunction with another dental insurance plan or offer.
• Dental Membership cannot be used for services covered by lawsuit, workers compensation, hospitalization or hospital charges, or cost of dental care covered under auto or medical insurance.
• Dental Membership Plan is for one year beginning on the enrollment date.
• Family members include children under 19 years old and their parents (or legal guardians).
• Family members cannot be substituted for other family members.  Enrollees name must be stated at time of membership activation.
• Dental Membership Plan benefits are not transferable to another dental practice or dental specialty practice.
• Dental Membership Plan activation fee is payable in full upon enrollment and is not refundable.
• Dental Membership Plan activation fee is payable in the first year only.  Activation fee is not charged in future years, even if the patient shifts into a different membership category, as long as the patient continues to pay monthly fee.
• Additional activation fees are waived after the first family member has paid the activation fee.
• Dental Membership Plan will automatically renew on the anniversary date, unless a written request to cancel is received 14 days prior to the annual renewal date.
• A 5% Membership Plan Discount is available for prepayment of the sum of all monthly fees for upcoming year if paying by cash or check.   5% discount is not available for activation fee.
• Payment for procedures or products not covered by the Dental Membership Plan is due at time of service, or at the first visit for multiple appointment procedures, in order to receive discount.
• Recurring monthly Dental Membership Plan fees will be due on the first business day of each monthly period closest to the enrollment date.  Credit Card payments are accepted and will be automatically withdrawn.
• Refunds will not be issued if the participant does not decide to utilize the Dental Membership Plan. It is the sole responsibility of the participant to utilize benefits by arranging the appropriate appointments within the 12 months of the membership period.
• Dental Membership Plan discounts on procedures do not apply if paying by Care Credit or other third party financial arrangements.
• Dental Membership Plan rates may be reviewed and adjusted on an annual basis.
• Dental Membership Plan does not include Cephalometric, Panoramic, Cone Beam, or Full Mouth Series Radiographs. The rationale is because these types of diagnostic imaging are not annually routine.  However these procedures do qualify for the Membership Plan procedures discount. Full Mouth Series Radiographs are included in the New Patient Experience option.
